Steps YOU must take to obtain an appointment with ACCUDOC doctor:

  1. Complete the Intake Form.
  2. Complete the Appointment Agreement:
    • Agree to Terms
    • Submit Medical Records
    • Pay for ACCUDOC appointment
  3. Once we receive ALL OF THE ABOVE we will call you to schedule your appointment.

Following your evaluation, your AccuDoc doctor will issue a recommendation.

When the recommendation is approved, the Ohio Board of Pharmacy will issue your Ohio Medical Marijuana card. You will use it at an Ohio Medical Marijuana dispensary.


After your appointment is complete

Please visit the MMCPR Registry at com.ohio.gov to finish paperwork with the state of Ohio. The helpline phone number is 833-464-6627.

  • Visit https://www.ohiomedicalmarijuanaregistry.com/logins/sign_in
    1. This is the website you will log into and retrieve your card. It requires you to sign in with your email address and password. If you have forgotten your password, there is a “Reset Password” link that you can use to make a new password.
    2. After you log into the system, you’ll see a “Renew Card” button near a picture of your card. Click on that and it will take you to the payment window, where you can enter your credit card information to renew. (This is a separate fee from AccuDoc’s appointment fee. This goes to the State of Ohio.)
    3. Once you have paid, you’ll be able to download or print your card out. If you’re doing this on a smartphone, you can take a screenshot of your card.
    4. If you have issues with your email address (you can’t log in or you don’t remember what it is), you can give us a call (812-932-3839) and we can change your email address in your account, and the links can be sent to your new email address.
    5. If you are a veteran or indigent, you can mail your DD214 or Disability paperwork to the state to get a discount on the price of your card as well as at the dispensary.
